Medical health plans are required for every student who needs to keep medication at school for ANY reason.
This may include allergies, asthma, diabetes, celiac, seizures, epilepsy, ADHD and more.
No medication can be dispensed, accepted or stored at school without the appropriate documentation being filled out by the parent/guardian as well as a medical provider.
Students joining us in Kindergarten are required to have a physical on file, with up to date vaccines as well as a dental and vision exam. These are required to be turned in no later than October 15th, they can be found here!
2nd Grade
2nd grade students are required to have a dental exam on file, it is available here!
